Canceling a Medical Record Request

Canceling a medical record is time sensitive as-to-not incur a cancellation fee from Release Point. If they have already started processing the request, you will incur a cancellation fee.Instructions

To cancel a medical record, proceed as follows:

• Click on the provider’s name that you want to cancel the record from

• Click the “Contact Customer Service” link

Click the “Select Contact Reason” radio button

Open

This will allow you to choose “Cancel This Request”

• Indicate the reason you are cancelling this request in the Notes section.

Click “Submit Inquiry” 

Open
