Making a New Medical Records Request

To request a new medical record order, follow the steps below:

Instructions

Click the cases tab 

Find the claimant this request will be made for

Click their name which brings you into their Case Details page 

Click the Medical Records tab

Click Request medical records button 

Properly fill out the patient information that has not already populated

  • Date of Birth

  • Choose Carrier & Source code if not pre-filled in

There may be an instance that you have more than one option to choose from and this affects billing actions to your firm.  These codes were established during the initial account setup with Release Point.

**Leave Policy/Claim/PID # blank

NOTE: Rush Order will incur a cost and will put this record order to the top of your requested records.  According to Release Point, orders are fulfilled in the order in which they are received, and only as fast as the providers return the request.

 

Begin typing the Provider’s name in the field provided.

  • The fields will populate with Release Point’s expansive database of providers that match your search criteria.

  • This request can be from any provider in the U.S.

  • Choose the provider associated with the claimant

If the provider is not listed, you can enter their information and select “Create Provider” Button

Perform this for all the providers you will be requesting records from

Click “+” to add multiple providers to this request

Choose the date range associated with this request

Either leave the “To” Date as-is and it will default to Present, or click change to adjust the date range

Upload required attachments

  • 1696

  • Authorization Form

Facility Specific Authorization Form

Click the Browse button to add a document

  • You can add multiple documents to this request by clicking the + Add Attachment button

NOTE: These documents will be attached to all of the requests made to each provider.
