How to Cancel Medical Record Request

Canceling a medical record is time sensitive as-to-not incur a cancellation fee from Release Point. If they have already started processing the request, you will incur a cancellation fee.Instructions

1. To cancel a medical record, proceed as follows:

   1. Click on the provider’s name that you want to cancel the record from

   2. Click the “Contact Customer Service” link

      

   3. Click the “Select Contact Reason” radio button

      

   4. This will allow you to choose “Cancel This Request”

      1. Indicate the reason you are cancelling this request in the Notes section.

   5. 

   6. Click “Submit Inquiry”
