Completing Required Hearing Forms

In addition to loading specific forms to the hearing, firms must also complete the Required Hearing Forms Section

Pre-Hearing Information

Ten days prior to the hearing, all of the information on the Pre-Hearing Information list must be completed and verified by the Firm. After the information is verified, press the “Save Pre-Hearing Information” tab.

Initial Contact

The attending attorney must contact the claimant three weeks prior to the hearing. The attending attorney must verify all of the information on the initial contact form with the claimant during the call, making updates, as needed. After all information is verified, it can be saved by pressing the, “Save Initial Contact”. The information can also be printed by pressing the “Print” button.

If the claimant cannot be reached, the attending attorney must indicate that on the form, make note of all attempts, and continue to reach out to the claimant.

Additional notes can also be made on the Initial Contact form, such as whether outstanding medical records exist and whether there is information that the Firm may need to know prior to the hearing.

Pre-Hearing Checklist

The attending attorney must make contact with the claimant a second time one week prior to the hearing and complete the Pre-Hearing Checklist. Information on the Pre-Hearing Checklist must be validated a second time. After the verification, press the  “Save Pre-Hearing Checklist” information.

Post Hearing Summary

After the hearing, the attending attorney has 72 hours to complete the Post Hearing Summary.

The Attending Attorney should add any information that is relevant about the hearing.

 

 

After the Post-Hearing Summary has been completed, click the “Save Post-Hearing Summary” button at the bottom of the last form.

Attorney Feedback (Optional)

After the hearing has been held the firm has the option to provide Attorney Feedback for the Attending Attorney.

Monitoring Required Hearing Forms

All required hearing forms can be managed from the Hearing Details Screen. Not only are there links to each form, but as the forms are completed, a check mark will be put next to each completed form. 

 

Hearing Flags

The Hearing Flags section allows for a quick evaluation of the status of each required form and process. Checkmarks will denote if the task has been completed; X’s will denote that the task has not.
