Prevail 10 E-Training* Questionnaire Setup
Questionnaires in Prevail allow you to guide a prospect or client through a series of questions. Once completed, Prevail automatically generates a matter or prospect using the collected information.
Questionnaire Setup enables you to customize questionnaires to fit your firm’s specific needs. Building a questionnaire from scratch gives you full flexibility to tailor the process and capture exactly the information that will maximize your firm’s efficiency.
Part 1: Questionnaire Types
There are two types of questionnaires you can use in Prevail:
Intake Questionnaire
Allows you to follow a script while speaking with a prospect
Once completed, Prevail creates a new Prospect or Matter.
Update Questionnaire
Also follows a script of questions
Used to update an existing Matter or related party fields.
Note: Prevail saves the full text of every completed questionnaire on the History tab of the Prospect/Matter.
Part 2: Setting Up Your Questionnaire
Step 1: Navigate to Setup
Go to Administration > Questionnaire Setup.
Click Add.
Step 2: Choose the Questionnaire Type
Intake Questionnaire: Creates a new Prospect or Matter.
Update Questionnaire: Updates an existing Matter.
Step 3: Enter Basic Information
Subject: Name of the questionnaire.
Category: Select where the completed questionnaire will be stored in History.
Notes: Admin-only notes about this questionnaire.
Step 4: Create the Initial Question
Every questionnaire starts with an Initial Question, which captures key contact details.
These fields include:
Office / Company / Firm
Title
Nickname
Date of Birth
Salutation
Gender
First, Middle, Last Name
Suffix
Step 5: Save
Click Save to confirm setup.
Part 3: Building Your Questionnaire
The main building page is split into three sections:
Field Codes | The prompts which will be added to the questionnaire. |

Question List | This is where you will drop the field codes to add them to the Questionnaire. |
Settings | This is where you will type in scripts and specify certain settings for the codes, which will vary depending on the code you are clicked on. |
Step 1: Understanding Prompt Types
Question | Best for open ended questions. When a question prompt is run in a Questionnaire, it will appear as a simple textbox to type in the answer. | 'Explain how your disability affects your everyday life' |

Pick List | A multiple choice question Pick Lists enable you to create branching paths within a questionnaire. Each answer can lead to a different outcome—such as ending the questionnaire or moving to another question.

Finish | These are used at the end of the questionnaire to tell Prevail whether to create an open prospect, closed prospect, or closed window. |

Step 2: Adding Prompts
To add prompts, drag the prompt type from the left pane to the Question List.
Part 4: Required Information
In order to use a questionnaire, you must add the following codes. They provide the proper information for Prevail to populate a matter or prospect.
Note: You can save a questionnaire without this information, but you will not be able to use the questionnaire until this information is input into the questionnaire.
Step 1: Set Law Type
A law type must be set in order for Prevail to create a prospect/matter:
Click the arrow next to 'Law Types’
Click the arrow next to the law type that pertains to this questionnaire
Drag the ‘Set Law Type’ prompt to the Question List
Use the dropdown to select the client type
Step 2: Assign a Security Group
Since each matter must belong to a security group, a Questionnaire cannot be saved until a security group is set.
Click the arrow next to 'Law Types’
Click the arrow next to the law type that pertains to this questionnaire
Drag the ‘Group Security’ prompt to the Question List
Override Prompt:
Check the override prompt checkbox if you want this questionnaire to default to the same security group every time it is run.
Use the dropdown to select the corresponding security group
Tip: Most firms only use the security group titled Default Group. This is a good reason to use the override prompt checkbox, so users don’t have to select the same group every time.
Step 3: Add a Finish Prompt
As mentioned above, a finish determines what you would like Prevail to do with the information once the questionnaire has been run.
Drag a ‘finish’ prompt to the Question List.
Use the Action dropdown to select if you want this information to be populated in an:
Open Matter
Closed Matter
Open Prospect
Closed Prospect
Close Window: This exits the questionnaire and does not save the information into a prospect or matter.
Part 5: Tips and Tricks
Set up a Pick List to go multiple directions
Once you have your pick list questions in place, drop another prompt on the answer you want it to correspond with.
Example: Pick List Question: Are you currently being represented for this claim? If they answer yes, you can drop a ‘finish’ prompt, ending the questionnaire.
Add scripts to prompts
Use the text box in the settings pane to type what you would like your screener to say for that particular prompt.
